Post Call Survey Monthly Drawing Terms and Conditions

1. Eligibility: The monthly drawing is open to customers of CleanSky Energy who complete a post-call survey after interacting with our customer service team. To be eligible for the drawing, customers must complete the post-call survey in its entirety. If a customer starts the survey but does not complete it, they will not be entered into the drawing. Only customers who complete all required questions and submit the survey will be eligible to win. Participants must be 18 years or older and residents of the United States. Only one entry per customer per month is permitted. Employees, vendors and contractors of CleanSky Energy and their immediate family members are not eligible to participate in the drawing. Void where prohibited or restricted by law.

2. Drawing Period: The drawing period will begin on the 1st day of each month at 12:00 AM EST and end on the last day of the month at 11:59 PM EST.

3. Entry: To enter the drawing, customers must complete the post-call survey in its entirety during the drawing period. Partial completions of the survey will not be considered a valid entry. Customers must answer all required questions and submit the survey to be eligible for the drawing.

4. Prize: One winner will be randomly selected each month to receive a virtual $100 Visa® gift card, which will be emailed to the winner’s email address on file at the time of enrollment. The virtual Visa® gift card is subject to the terms and conditions of the card issuer.

5. Drawing Date: The drawing will take place on or about the 5th day of each month. The winner will be notified via email within 7 days of the drawing date, and the prize will be emailed to the winner shortly thereafter. The winner will be selected through a random drawing conducted by CleanSky Energy.

6. Odds of Winning: The odds of winning depend on the number of eligible entries received during the drawing period.

7. Taxes: The winner is responsible for all applicable federal, state, and local taxes associated with the prize. The winner may receive a Form 1099 if required by law.

8. General Conditions:

• By participating in the drawing, customers agree to these terms and conditions.
• The drawing is subject to all applicable federal, state, and local laws and regulations.
• We reserve the right to modify or cancel the drawing at any time without notice.

9. Governing Law: These terms and conditions shall be governed by and construed in accordance with the laws of the state of Texas and venue shall be in Harris County, Texas, without giving effect to any principles of conflicts of law.

10. Dispute Resolution: Any disputes arising out of or related to the drawing shall be resolved through binding arbitration in Harris County, Texas in accordance with the rules of the American Arbitration Association.

11. Additional Terms:

• The virtual $100 Visa® gift card prize is non-transferable and cannot be exchanged for cash or other prizes.
• We will attempt to contact you at the phone number indicated on our telephony system or provided by you to provide you with a virtual card. We will also send the virtual card to the email address on file or provided by you. It is your responsibility to ensure that your contact information is accurate and up to date.
• We are not responsible for any technical issues, including, but not limited to, email delivery failures, that may prevent the prize from being delivered to the winner.
• The $100 Visa® gift card will be sent in the form of a digital gift card and can be used anywhere Visa® is accepted.
• The prize will expire on the date indicated on the digital gift card, and any unused balance will be forfeited.
• We reserve the right to substitute the prize with one of equal or greater value if unavailable.

12. Customer Information:

• By participating in the drawing, customers agree to allow us to use their personal information, including their name and email address, for the purpose of administering the drawing and delivering the prize.
• We will not share customers’ personal information with any third parties, except as necessary to fulfill the prize.

13. Drawing Administration:

• The drawing will be administered by CleanSky Energy, whose decisions are final and binding on all matters related to the drawing.
• We reserve the right to disqualify any customer who tampers with the process or violates these terms and conditions.

14. Limitation of Liability:

• By participating, customers agree to release and hold harmless CleanSky Energy from any claims or damages.
• We are not responsible for injuries, losses, or damages from participation or prize acceptance.
• We are not responsible for technical issues that may prevent entry or completion of the survey.
• We are not responsible for administrative errors in the drawing process.

16. Severability:

• If any provision is found invalid or unenforceable, the rest shall remain in effect.
• The invalid provision will be deemed severed, and the rest construed accordingly.

17. Headings:

• Headings are for convenience only and do not affect interpretation.

18. Acceptance:

• By completing the post-call survey, customers agree to these terms.
• Customers not in agreement should not participate.

19. Additional Provisions:

• We may update these terms at any time without notice.
• It is your responsibility to review terms regularly.
• Continued participation implies agreement to changes.

20. Acknowledgment:

• By completing the post-call survey, customers acknowledge agreement to these terms.
• Customers consent to participate and receive the prize if selected.
